The 6.5 quake damaged the building, caused landslides, and caused people to take to the streets.
The shocks were registered Tuesday evening, based in a mountainous region in northern Afghanistan, near the Pakistan border.
The sways from the remote Yarm Valley felt as far as India.
It was a terrible shake. I had never felt such a shake before in my life,” said Kabul resident Khatera for the AFP news agency after rushing out of her fifth floor apartment.
Nine of the confirmed deaths were reported in the Khiber Paktunkhwa Valley region in Pakistan.
Three others lost their lives in Afghanistan, the country's Health Ministry spokesman said.
A child was among the dead in Leghman province near the country's border with Pakistan, AFP reported. Many families had left their homes celebrating the Persian New Year or Noruzi when the earthquake struck.
The rugged distance and terrain of affected areas will likely slow down relief and rescue efforts.
Phone lines have been affected, and in Pakistan the highway in the hardest hit area has been blocked by landslides.
Pakistan's Prime Minister Shehbaz Sharif has urged disaster agencies to take urgent measures to help people.
The shocks were felt in a 1,000km area involving India, Uzbekistan, Tajikistan, Kazakhstan, Kyrgyzstan, Afghanistan and Turkmenistan, according to the European-Mediterranean Seismological Centre.
