You don't mark the gender on the booklet. The reason will surprise you.

Georgia: Unknown. The health booklet of Searyl Doty, a Canadian child born in November 2016, has caused much controversy in the country. This is because his parent, a transsexual, has decided that his child will choose for himself in the future which gender he will inherit. Canadian media report that the child was born, not in a hospital, but in an apartment just to keep his gender in his health card. Authorities in the country have again refused to issue a family certificate until the precise gender of the child is established.

For this reason his mother has turned to the appeals court, receiving a document from the health ministry. But in such situations Canadian provinces may maintain different attitudes: In Alberta people can put X to the gender index, while in Ontario you are allowed to choose the neutral gender for health licenses and driver's patents.
