Sunderland has started talks with Germany's champions on a deal that reportedly amounts to 10m euros, reports Telegraph, broadcast Perixopi.
Also, the English club is willing to offer 32-year-old a three-year contract to convince it of this new adventure.
Sunderland climbed to Premier League via play-off and is calling for experienced reinforcements to ensure survival.
For Jaka, this would be a beautiful return to England, where he spent seven years with Arsenal before moving to Leverkusen.
