Magdalena Andersson's centre-left coalition appears to be narrowly lost to a right-wing party block, 176 seats against 173, with 99% of the votes counted.
The leader of the Fashion Party, Wolf Cristersson, is now expected to form the government.
The bloc includes Sweden's Democrats, a far-right party that has campaigned against increased gang shootings.
The final result remains to be confirmed after a recount, which is standard practice in Sweden.
But despite that, Andersson conceded defeat at a press conference Wednesday and said he would officially resign Thursday.
“in parliament, they have one or two front seats”, she said. “It's a thin majority, but it's a majority”.
The tough election campaign was dominated by gang issues, immigration and integration, as well as rising electricity prices.
Andersson was the first female prime minister of the Nordic country when she took office last year. She resigned on the first day before returning immediately.
