Germany has triumphed against the Serb in the semifinal of London's “Atop London”, in three sets of 7-6 4-6 6-3.
It was German's fourth career victory over Djokovic, in the face of seven Serbian victories in direct confrontations.
Zverev joins the finals Daniil Medvedev, who in the semifinals had defeated Caspian Ruud.
Last year's winner was Medvedev, who had defeated Dominic Thiem in three sets, while Russian tennis aims to triumph for the second consecutive year.
The most successful player in this tournament is Roger Federer, who has won it all six times, while the second ranks Djokovic with five London titles.
