The Chicago-born star whose real name is Pigick Coleman died in his home in Reseda County Los Angeles.
Fredo had problems with his liver and kidney, while he had confirmed this during October at Instagram, where he said he was being treated for diseases of these organs.
Santana was a cousin of the Chief Keef (the real name of Keith Cozart), best known to the new generation of Chicago rappers who have recently infiltrated the scene there.
Keef's grandmother, Margaret Carter, said Fredo had been sick, but he had not talked to his family about his illness.
Repper and his girlfriend had become a boy's parents last June.
Chief Keef expressed grief at the death of his cousin, saying they had big plans together.
